Types of care

We specialise in providing good quality care for people living in their own home. We offer a unique service of tailored visits from 1 hour to overnights to full live-in care and support. We enable people to remain at home where they are most happy, comfortable and relaxed.

Whilst our primary services involve the day-to-day care of people in their own homes, we also provide a range of specialist services which include: companionship to all aspects of personal care and specialised care, such as assistance with medication, PEG feeds, tracheostomy care and oxygen management. We also offer overnight stay packages between the hours of 10pm and 7am so you can recharge your batteries with a proper night’s sleep, and live-in care for people requiring a longer break. We will tailor our service and staffing provision to meet your needs.

Dementia and Alzheimer’s care

Many people with dementia are looked after on a day to day basis by a spouse or other family member. However, caring relatives also need a break from time to time and this is where our ‘dementia care service’ can help.

On an arranged day or days each week Crossroads will come and take over the caring role allowing the caring person to have some time to themselves. All Crossroads staff have received specialist dementia training which is regularly updated through our in-house dementia champion so all our staff understand the practical and emotional aspects of caring for people with dementia.

Live-in care

Crossroads Together live-in care services provide a longer break for the caring relative.

We can provide support around the house at various times of the day and night to carry out varying tasks. No two days need be the same and we can tailor a plan to include different tasks each day. This service is more cost effective than residential care and will enable people to remain in their own homes in a familiar and comfortable setting.

Learning disability care

All Crossroads Together staff are highly trained, with extensive practical experience across many different areas of learning disability. These include behaviours that challenge, autistic spectrum disorders,complex health needs, physical disabilities and sensory impairment.

Our care is wide ranging and flexible, from personal care through to support in leading an independent life. This can include a day trip out or a trip into town for shopping, help with socialising and meeting friends, or pursuing a sport or hobby.

End of life care

Crossroads Together can help those with a terminal illness or terminal condition that has become advanced, progressive and incurable.

Crossroads Together ensure that people are comfortable and supported within their home environment. We have many years’ experience working with local health teams and professionals to ensure a co-ordinated and seamless approach to care. Choice and control are at the centre of Crossroads Together care planning and delivery. Our care is delivered in a sensitive, person-centred way, taking account of circumstances, wishes and priorities of the individual, family and friends.

Children’s Care

Crossroads Together provides services that protect, care for and support children with disabilities.

It may be that care is needed while a parent or relative is ill or in hospital. Alternatively your child could need more specialised care following an accident or illness. Our support enables children with disabilities and challenging behaviour to develop and grow into adulthood with the specialist assistance they require. Supporting children to grow into the best person they can be is not only beneficial for the child but also for their family, and is highly rewarding for our staff.

Respite Care

Crossroads Together can provide a planned programme of respite care visits, from an hour to an overnight, for a few hours through to a few weeks, for families and those providing unpaid care.

Even though many families take great joy in providing care to their loved ones so that they can remain at home, the physical, emotional and financial consequences for the family member providing care can be overwhelming without some support, such as respite care. Crossroads Together respite provides the much needed temporary break from the often exhausting challenges faced by the carer, giving the carer an opportunity to take a break or holiday, relax and recharge.
