Requirement to travel in the community of Cheshire East
Based at Overton House, Congleton
Part-time 22.5 hours (3 days per week)
Fixed Term until 31st March 2023
£20,085 (Pro-rata)
We have an exciting opportunity for two individuals to be linked to a project funded by Cheshire East Council. You will need to promote social inclusion and increase the independence and self-esteem of people using our services by helping them identify and achieve their goals.
This role will involve providing community based groups/sessions and promoting access to a range of activities within the wider community. It will also involve working with the Cheshire East community network to develop and support a range of enjoyable, interesting and relevant social activities, you will need to manage your own caseload and provide each individual with a personal centred approach.
As well as a good understanding of mental illness and the long-term effect COVID 19 has had on people’s increased social isolation and loneliness, we will be looking for the ability to prioritise a busy workload and work well within a team.
You will also need the communication skills it takes to, motivate and inspire others, and liaise effectively with a wide range of organisations and individuals. You will build trusting relationships with people at all levels, create a shared personalised care and support plan and connect them to community groups , Voluntary organisations and services as well as work with partners to provide support to organisations involved in social prescribing.
You must be a good listener, have time for people and be committed to supporting local communities to care for each other, also have experience of working positively with people facing complex social and emotional challenges as well as have great interpersonal skills and ability to support people, community groups and local organisations.
Each day will be different, meeting people referred to the service with a range of health, social, emotional, practical needs, all with different goals they wish to achieve; requiring you to offer personalised support.

About You
Experience of using IT packages including, Word, Excel and Microsoft Teams |
Excellent organisational, communication skills |
Experience with monitoring and reporting outcomes |
Confident in public speaking, having experience of presenting ideas or feedback to groups (large and small) |
Ability to motivate self to work within deadlines |
Experience of partnership working |
Ability to travel across locations and work flexible to meet the needs of the services |
Ability to develop and promote positive working relationships with customers, professional and colleagues |
Full UK driving licence and access to a vehicle are essential |
