Monday-Friday, 22-25 part-time hours (flexible working considered for the right candidate)

We are looking to recruit an experienced and dedicated Care Coordinator to join our Greater Manchester team, based in Bolton. You will be an integral part of a busy and proactive team, ensuring that the highest level of care is delivered to our clients.
You will be responsible for leading and motivating a team of Carer Support Workers
Carrying out assessments and formulating person centred care plans.
The ability to prioritise and multi-task are key to this busy role.

About You
It is essential you that you have experience within health and social care sector |
You must have a QCF/NVQ Level 3 in Health & Social Care or working towards this qualification |
Excellent organisation and communication skills |
Excellent IT and Computer skills |
Previous experience of creating/managing work schedules would be advantageous |
It is essential to hold a full UK driving licence and access to a vehicle as travel around Greater Manchester is essential |
